Former Employee Contact Information Update Form

In February 2024, the City on a Hill Charter Public School Board of Trustees voted to return the School's charter to the Commonwealth and cease operations effective June 30, 2025. As the School prepares for this closure, we are seeking updated information from former staff members to be able to provide them with documentation relating to employment verifications and retirement fund(s).

If you are a previous staff member of any school within the former City on a Hill Network (Circuit Street, Dudley Square, New Bedford) during any time since their founding, and your contact information has changed since you departed, please fill out the following form to recieve these important updates. Following the School's closure in June 2025, the Department of Elementary and Secondary Education (DESE) will retain these notices.
